How do I access my health information?

Simply contact the Health Information Management department at (509) 576-4340. To submit a Release of Information, simply click the link below to download, print, and complete this form.

How do I contact my provider/clinician?

Simply give us a call at (509) 575-4080 and let us know who you need to speak with, and we can transfer you. You can also call the facility or site directly. A list of our locations can be found here. Please feel free to leave your provider a message as they might be in a session with another client at the time of your call.

 

What if I don’t have insurance?

First and foremost, crisis calls are free of charge to anyone regardless of insurance. If you do not have insurance, and would like to begin services with us, one of our In-Person Assistors can help you with the enrollment process to Washington Healthplan. Click here to learn more.

This website has a lot of information on a variety of diagnoses like depression, anxiety, bipolar disorder, psychosis, and addiction or substance use disorders to name a few.
